I've seen both end of this spectrum where done is not defined enough, and where done is defined in a many page document. Often times it seems that features are too big to easily describe and turn into stories. This is where the Product Owner training comes into play, and why the certification exists. Taking vague requirements from customers is not easy, and making those into testable user stories with acceptance criteria is even harder. By reducing the complexity and creating more workable chunks, it does become easier.

I was glad to see the cartoon "Make Mine Freedom" ...

I was glad to see the cartoon "Make Mine Freedom" on your web site. "I know a company using it, and it's working for them, so it should work for us, right?".

That's the main issue right now, and currently there's a barrage of articles on how Agile is helping multi-billion dollar companies, there's a huge hype building on. I have published recently an article about agile limitations, which anyone who actually wants to get into agile will find it a good read.
